The exception to this test is the rare condition of dexamethasone- suppressible hyperaldosteronism in which plasma aldosterone levels in the patient fall on standing. This diagnosis can be confirmed by demonstrating that the blood pressure is reduced to normal by ACTH suppression. If patients with adenomas are stressed by standing, then ACTH levels, and hence cortisol levels, will rise during the course of the morning. Furthermore, aldosterone levels will also rise. If this occurs, the test will need to be repeated.
